A Clinico-Statistical Survey of Ectopic Pregnancies Following Previous Tubal Sterilization Surgery

Abstract
It is now evident that majority of pregnancies following previous tubal sterilization surgery(P. T. S. S) are ectopic pregnancies. So, this investigation reviewed retrospectively 26 cases of ectopic pregnancies following P. T, S. S among 397 cases of ectopic pregnancies which were admitted & operated in Dept. of Obstetrics & Gynecology at Soonchunhyang Univer-.sity Hospital from March, 1975 to May, 1983.
The results of this study were as follows:
1. The rate of ectopic pregnancy following P.T.S.S among total ectopic pregnancies was 26/397(6. 5%) and tended to increase annually.
2, In the group with P.T.S. S, the most frequent age group was 35?39 years of the age (38.5%), the mean age was 34.5 year. But, in the group of ectopic pregnancies without P.T.S.S, the most frequent age group was 25?29 years of the age(30.4%), the mean age, was 30.2 year. The frequent age group of ectopic pregnancies without P. T. S. S was significantly younger than that of following P.T.S.S.
The mean numbers of children in the group with P. T. S: S were 2.5. The mean numbers of children in the group without P.T.S.S were 2.2. There was no significant difference berween both groups.
.4. Of the total 26 cases of sterilization failures, 15cases(63%) were revealed to have occured within 4 years after their surgery. The mean duration was 3.8 years.
5. In 19(79%) of 24 cases of the group with P. T . S. S, the ectopic pregnancies were located in the distal part of the tube, in 5(20.8%) of the 24 cases, in the proximal part. In 290 (80.4%) of 361 cases of the group without P.T.S.S, the ectopic pregnancies were located in the distal part, in 61(19.7%) of 361 cases, in the proximal part. Both groups were not significantly different in the location of the ectopic pregnancies.
o: The most frequent duration between LNMP and ectopic pregnancies operation was from 5 to 8 weeks (38.5%) in the group with P.T.S.S. and 5 or less weeks (56.9%) in the group without P. T. S. S. , and showed statistically significant differences between both groups.
7. The pregnancy test with urine(Gravindex) was pregnant in 47.6% in the group with P. T. S. S? , and 41.7% in the group without P. T. S. S. , and showed no statistical differences between both groups.
8. The culdocentesis was positive in 86.% of the group with P. T.S.S. , and in 95% in the group without P. T. S. S. , and showed no statistical differences between both groups.
9. The mean blood loss during the ectopic ¢¥pregnancies operation was 937 ml in the group with P. T. S.S. , 1,100 ml in the group without P. T. S. S. , and showed no statistical differences between the both groups. The chief complains of the ectopic pregnancies were abdominal pain, nausea & vomiting and vaginal bleeding or spotting, and were similar in both groups.
10. The operative managements of ectopic pregnancies following P. T. S. S were ipsilateral salpingectomy($0.8%), and in 21.9%, contralateral tubal ligation was combined.
